1. Application for admission to specialist accreditation, which is filled out on the computer, printed out and signed with a black gel pen
2. Identity document (passport - pages 2, 3, page with registration)
3. Insurance certificate of mandatory pension insurance 
4. Documents will be accepted until 07/05/21, Monday through Saturday, from 9 a.m. to 4 p.m.
